As a courtesy to our patients, we will file your dental insurance forms. As you know, each insurance company has rules governing coverage as well as fixed amounts for certain procedures, and most companies have an annual deductible and a maximum they will pay per year.
While we will file dental insurance forms for our patients, we want you to recognize that it is the patient’s responsibility to pay any deductible, co-insurance, or other balance not paid by your insurance company.
